For many of our patients, it is important to keep your appointment. If you have questions in regard to delaying your appointment or surgery, or if you have been sick or been exposed, or awaiting COVID-19 test results, please contact our front office staff at 772-567-1164 option 5 before coming to our office.
Our top priority is the health of our patients and staff. We follow all precautions and recommendations by the Centers for Disease Control to provide a safe environment.
Patients will need to answer several health questions and sign a Health Self-Declaration. You will be asked these questions when scheduling your office visit.
We are working to minimize the number of people coming into our office, so we ask if possible do not bring a guest or to have them wait in the car for the duration of your appointment.
The CDC now recommends that the general public wear a cloth mask when leaving the house.
